The Sunfall Axe is found in Stone Titan Caves in Abrassar. To obtain the axe you must read all the Rune rocks (four of them) within the cave. Once you've read them all; head to the bottom of the cave to an altar with a fifth rune rock in the middle of it; the axe will be on the rune rock.
