Obtained in the Face of the Ancients dungeon in the north-east region of Enmerkar Forest.
Defeat the First Cannibal, located in the frost cave part of the dungeon, and obtain the key that it drops. Use this key to unlock both doors in the central frost cave. One door is a shortcut to fire part of the cave, while other doors lead you to a small room with one corpse and Scarlet Lich's Idol. Return to the Red Lady's Altar in the fire cave and place the idol on it. The dagger will spawn on the illuminated plate in front of the altar.
